The person who understood the system leaves, and what's left behind becomes unreadable.
What it looks like
- —Only one person, in-house or at a vendor, ever really understood how it works
- —There's no documentation beyond what's in that person's head
- —Any change now requires 'figuring out how this even works' before anyone can touch it
Why it happens
Documentation and handover feel like overhead during a build, and they're the easiest thing to cut under time pressure — until the person who built it is gone and the debt comes due all at once.
How to avoid it
Ownership transfer isn't a checklist item at the end — it has to be built into how the system is documented and structured from day one, so more than one person can read and run it at any point.
