Hallucination
When an AI model generates confident-sounding but factually incorrect or invented information.
Because LLMs generate the statistically likely next words rather than looking up facts, they can produce plausible-sounding but false statements — inventing citations, numbers, or facts. Grounding answers in retrieved source material (RAG) and explicit verification steps are the main mitigations, not a full fix.
